A Core Interactive incident investigation course delivering 11 interactions and 7 validated competencies across securing the scene, gathering evidence, interviewing, and tracing root cause past the obvious. Findings drive corrective action rather than blame.
An incident that is investigated well is an incident that does not happen twice. This course teaches supervisors, safety-and-health committee members, and workers how to conduct an effective incident investigation — to find the causes and prevent recurrence, not to assign blame. It covers why and when an investigation is triggered (serious incidents and significant near misses) and who should be involved (the supervisor together with a worker/committee representative); how to secure the scene and gather facts from the four sources — the "4 Ps": People (witnesses), Position (the physical scene), Parts (equipment and materials), and Paper (procedures, training, and maintenance records) —…
Equip supervisors, committee members, and workers to conduct an effective, no-blame incident investigation — secure the scene, gather the facts, find the real (system) causes, and drive corrective actions that prevent recurrence.
